This does not look like a js error to me, specifically it says that the <label> and the <element id> are mismatched. imo you should be reaching out to Formidable Forms support.

Where are you seeing a js error, and what is the error?

Did you dump out the Formidable Forms, and try a different form plugin?

I will try the plugin you mentioned tomorrow. Sounds like it might be the quickest and easiest option.
