How to integrate with custom form

Hope you can help.

I am trying to integrate a custom form with your plugin. The form is part of a logged in section where the user can refer 3 friends email addresses and they all get a gift afterwards. The emails are then stored as user meta.

We would like to then store the 3 email addresses in our Mailchimp list. Is there a way I could link this form to your plugin?

I found the below link which looks almost perfect however I am looking to add 3 email addresses not one.

The name attributes on the email inputs are currently email_address_1, email_address_2 and email_address_3

Do you have any ideas? It seem silly to do a custom Mailchimp integration when we have your great plugin installed!
