How to personalize a post using profile fields

This should be a fairly obvious idea for a plug-in, but I can’t find one (or maybe I’m just using the wrong terms in my search).

I would like to use the logged-in user’s profile fields in the content of a post. This would allow personalised posts, perhaps using shortcodes, with the post content. Perhaps a user clicks on a menu item “download your certificate” – with the page/post content looking something like:

Hello [profile field='name'], your certificate number is [profile field='cert-no'], which you can download <a href="/certificates/[profile field='cert-no'].pdf">here</a>

Perhaps someone can suggest where to look for such a plugin.
