How to re-add the page?

    Hi, I mistakenly removed the page, is there any way to re-make it as it was when I first installed the plugin?

    Hi,

    You can just create a page and add the shortcode to the content body [REQ_CRED_FORM].

    Worked wonders, cheers. It would be awesome if you could edit an option for the submit button text, so that you can adapt it to other languages ??
