How to remove the leave a reply/comment count link within a post

On the twenty twelve theme on the main page it displays my comment count at the bottom of new posts but when you click on an actual post, down at the bottom of the post under the social network share buttons it shows again and right under it is my disqus plugin with its comment count. Sometimes my disqus count and the comment count for wordpress doesn’t sync right away so one might say 9 comments the other might say 11 comments. How do i delete the ‘leave a reply/comment count’ within a post but keep it on the main page??
