How to revert to version 3 ?

    I was using version 3.5.52 and everything work fine but today i tried upgrade the plugin to version 4 but it didn’t work (no password shown). I reinstall ver 3.5.5.2 but it didn’t work either (no password, custom forms revert to defaults).
