You could do it that way, but I recommend the following snippet for customizable per-donation form emails: https://github.com/WordImpress/Give-Snippet-Library/blob/master/email-customization/per-form-email-content.php

We’re going to come out with a full UI for per-form emails via a Give Add-on, but until then this is how to do it programatically.

There is a lot of inline documentation in the function that describes what’s going on. Essentially, it is retrieving the donation form ID from the donor’s payment ID, then if there’s a match to our specific form we output whatever content, HTML, and/or custom data that we need. This should be what you’re looking for. Let me know if you need anything else.
