How to solve country field error?

I wanted to use country state city drop down in my contact form. It seemed very easy to set up but when I wrote the form tags for country, state and city drop down, only country drop down is loading, rest of the two are asking for Country Field availability. Even when the country field is already available still they are asking for it. Please help me with this.
