Title: You can write it how you like. The form will modify the title to use appropriate characters only.

Description: Write a description of the main functionality of your plugin. Have a look on the Plugin page for a plugin you currently use on one of your sites to get an idea of what to write.

Plugin URL: You need to provide a link to your plugin, which is in .zip format. You must pack a readme.txt inside the zip, an example is given here.
So put your plugin files in a folder titled according to the name of your plugin. Put your Readme inside. Use 7Zip or WinRAR to pack as a .zip and upload to somewhere on a site you run. Copy the link to the .zip into the URL of the WordPress Add Plugin form.
