HTML on Custom Fields & Nest form?

First of all, congratulations for this plugin. Great job =) but I have some difficulties and i will try my best to present my doubts.

I have a project in which this plugin fits like a glove, however, i need the options of custom fields appear customized, for example:

I have a dropdown custom field but in the first moment I need customize his css to add a background image and dimensions. After that, I need that instead of text in the options of the dropdown to be shown an image, or at least that I can add a link on the text.

I believe that in this second case if there is a way to allow html in the field of values would be perfect, but I have no clue how to do this, or if the plugin has such an option.

Moreover, still on custom fields, I am interested in developing what I believe to be called nest form. Basically, I want to create a dropdown with another dropdown in each of the options (believe with html release in the values I would be capable of that too). For example, I have a dropdown about food. When the user open will have these two options:

fruits
greens

Hovering the mouse over the options, or select, a new dropdown appears with more options:

fruits:
-Apple
-Watermelon
vegetables:
-Onion
-Tomato

There is a simple way to be developing this?
