Thanks for your report! Can you share the complete .htaccess file with us? Preferably in a Gist or via Pastebin. Also, what PHP version are you running on?

Before you ask:
%{HTTP_HOST}%{REQUEST_URI} and {DOCUMENT_ROOT} are working.

@flowdee I’m admitting this is pure guesswork, but have you tried to disable Apache Content Cache?
Also, if you permalink settings don’t include a slash at the end, the htaccess snippet would look slightly differently.
