I cant find the code for showing the form to request testimonials

Yes, that is possible.
First you need to create a view for the form (testimonial – views)

In the menu ‘testimonials – views’ you will see the shorcode of the form.
It should show something like [testimonial_view id=”x”]
where x is the number of the view.

Then you add the shortcode for the form in a page.
If you surf to the page it will show the form.
You can send then the url/link of that page to people.
