I need multiple terms checkboxes

I need to have a checkbox for both Terms AND a Waiver — so TWO check boxes for users to agree to different things. Ideally they would be separate because adding all that text makes the “Show Terms” extremely long. Also a way to link to another page (open in a new window) to view the terms would be great too!
How can I do that?
