it’s your theme that changes the padding of the sticky placeholder.

The good news?

You can keep the icons ??

Add the following code to the Custom CSS field in Optin Forms:
#optinforms-form3-container .sticky-placeholder-label{padding-left:33px!important;padding-top:6px!important;}

Hit save and re-load your page.
