Icons not displaying

I do not see the icons either in the Profile forms, or in the Dashboard. Obv I have some permissions or install issue.
I use UM to restrict access to pages and menu items. Here are my other plugins, all latest versions:
* Akismet Anti-Spam
* bbPress
* Child Theme Configurator
* Events Manager
* Jetpack by WordPress.com
* MaxButtons
* PayPal Buy Now Button
* Really Simple SSL
* Simple WordPress Membership [for Paypal subscriptions]
* WP Edit
* WP Universal Newsletter
Also, I cannot update the profile image.
I am guessing a folder has the wrong permissions, so if anyone can suggest what folder needs what, I’d appreciate it. At present all folders have 755.

I also redid the Profile form, which got me the three icons on the menu:
However, I still cannot display the icons or upload an image file.
