image description/caption/title

i have uploaded some images to the media library

how would i go about pulling out the image title, caption, description?
i need to put these into my site separately

i have a jquery portfolio that has separate places for the image, title, and caption.
i have got the image to display but that is it.
i dont want to hard code in the info every time

i have searched everywhere and can’t find an answer
