Image upload question

  • I know this question has been asked many times however I have followed all the advice and still cant fix the problem.

    I can upload the image, it shows up in the Media Gallery, but as soon as I click insert the window goes white and no file uploads.

    Fire bug gives me this error.

    Permission denied to get property Window.send_to_editor
media-upload.php?type=file&tab=gallery&post_id=27()()media-up...ost_id=27 (line 4)
[Break on this error] <meta http-equiv="Content-Type" content="text/html; charset=UTF-8" />

    I have followed every bit of advice I can find in this forum but cant seem to fix the problem.

    The problem started after an upgrade by my hosting company but there say it is not a problem they could have caused. Is this correct? is there any server setting besides mod_security that could effect media upload?

    Any new advice that is not already listed in the forum would be a great help.
