Image/Icons Size Issues

Hello Mike,
I was using the beta version and updated to 3.4.1 today and am having issues with the image icons being different sizes. I have used Small and Xsmall for all my pages but now the xsmall icon are just dots but only on some pages, other pages they are still smaller but bigger than a dot.
