Images not showing in "Insert Media"

  • Uploads are working absolutely fine, as is the media gallery, but when I go into a post or page and Add Media, I can’t see the most recent uploads.

    Images from about 4 months ago are there but nothing more recent, even though I can see them in the Library and the actual files on the server.

    All plugins disabled but no change. Does the Add Media gallery cache the list somewhere?
