    Hi Grzegorz,

    Please refer to the SEO Data Migration guide.

    Which will explain how to use a plugin to make it all work automatically for you (and error handled).
    Be aware that Yoast custom Titles (recently?) alters the whole title instead of just the page/post part.

    In 2.6.0 there will be a feature in the SEO Bar to check for this (duplicated title) and make you aware of such.
