Incorrect shipping price

My shop is based in the UK, offering flat rate shipping.
There are 3 shipping zones: UK, Europe, and everywhere else.
Flat rate is the only shipping method.
PayPal is the only shipping option. There are no shipping classes set up.Every so often I get a UK customer telling me their shipping price has been incorrectly calculated – it goes to the Europe rate instead of UK.
I have no idea why this might be. It only happens sometimes, and I can’t replicate the problem. It only happens for UK customers.
