Having said that, I have to say that in my experience at Wordfence, it has been very rare to see a site compromised and usernames changed overnight. That requires access to the database or at the very least, the wordpress admin area. It’s possible that this is the culmination of a compromise that happened a while ago and only now is being actively used. At any rate, this is only speculation since I don’t know the particulars of the site at all.
