Ink Appointment Booking Shortcode

I am using Ink Appointment Booking plugin for my website. But when i am placing a widget of Ink Appointment then it is messing up my site design.

So i used text to insert the form in the widget. But the form is not working dynamically. So is there any php shortcode for inserting dynamic form in widget..?
