inputs – different prices

    Anyone knows any plugin I can use to create 3 different price options (radio box) for customer to pick up. I want to redirect to new page with inputs to fill in when the user clicks one of the option. For example first option for ￡3 – one input to fill in, second options for ￡5 with 3 boxes to fill in and third option for ￡15 with 20 boxes to fill in. Then the customer should be redirected to payment page. Anyone knows how can I do that? Thank you
