Insufficient permission to use plugin as administrator

From the plugins managing page, after activating the plugin, I clicked the ‘settings’ link under the plugin and I get the wonderful message,
You do not have sufficient permissions to access this page.

Also, there is no “Appearance > Custom CSS” menu item.

I am the site administrator.
