    I posted in a thread marked resolved so I decided to put this in its own thread. I have definitely had functionality fail after some of the recent updates…

    I am also having problems with the plugin since some resent updates. I had no problems with it for a long time and then during a recent push for registrations we were inundated with emails from frustrated users who had the correct answer but validation would fail.

    It’s been rather hard to debug as the problem is intermittent but I believe I am close to an answer.

    I think setting the raw_output parameter to true when creating the $Salt in functions encode and decode are resulting in errors for some browsers. This seems especially true when the captcha answer is one of a larger value, say 9 rather than 2.

    In short I am able to replicate the issue with larger answers and the md5 raw output set to true. Setting it to false seems to make the issue go away.

    There’s definitely an issue with the plugin, I’ve been working on this for a couple of weeks and this is the closest I’ve come to certainty as to what is happening.

    There is most definitely some problem with the salt. When I simplify it (such as rolling it back to the hard coded salt) it works 100% of the times. I had many, many, many failures in Chrome with the md5 salt, no matter what the raw_output parameter was set to.

    Could you pleas specify your observations?
    1) we need a screenshot of the plugin settings page in both cases (when it works correctly and when the problem occurs).
    2) you may be using a plugin for caching. It may affect captcha’s work. Please try to deactivate all the plugins except captcha and check its work – will the error occur?
    3) When exactly did the error start to occur and what actions caused it? Did you install or update anything?
    4) Please provide a link where captcha works incorrectly and specify if it happens on all the forms or only on the comments, login, registration form?

    I was trying to update the captcha 3.9.X and got the error (below), Now I can’t even view any plugin on my site. Can someone please throw some light on this problem ?

    I don’t know if this has been ‘fixed’ as there hasn’t been a post for a while with the same problem (maths not being accepted). I think johnja1 has a different issue.

    However, we installed this plugin recently and are also having issues with incorrect refusals. These refusals appear random, but have occurred using using Chrome v33 and IE v10 (this is from our site’s visitors).

    We are using W3 Total Cache plugin. I don’t want to disable it as it is solving a different more serious problem. Also, as this is very widely used plugin, I think it would be fairly important for Captcha to be compatible, if possible. We like Captcha for its simplicity.

    We are only using it on the comments form at present.

    We are also using it in Portuguese, but I assume the translation is okay as numbers to words is so simple, but haven’t checked this.
