Internal Server Error

    Do you have a caching plugin running? I’m still new to WordPress, but I think I’ve seen caching plugins cause similar issues.

    No there is no caching plugin installed. But the form was down. I couldn’t even access it in the backend of the site, and when I attempted to go to their website formget.com there was also the same internal error. So I’m assuming it was an error on their site. It appears to be corrected now, but my concern is if this happens often. This form needs to be functional in order for people needing to request lodging to be able to do so and my client wasn’t too happy to suddenly see his form not functioning.

    Sorry for the late reply. That is definitely an issue. I use Ninja Forms I like it quite a bit. It’s easy to use and they have great documentation. I’ve never experienced that problem with them before.
