Internal Server Error's

We have used the plugin basically for 2 years with no issues, using the APC method, amazon CDN and cloudflare too.
I updated to the latest version about 10 days ago and since then we have had frequent internal server errors with the .htaccess file being somehow changed and to fix it, I have to paste in a working htaccess code, I don’t understand why or how it keeps changing the htaccess, even when using 444 CHMOD.
Has anyone else had issues like this? I tried googling this and all I got was the tmp folder needs to be 777 which it is, I thought this was fixed but then yesterday the whole cache folder disappeared and it all started happening again. Now today within a few minutes of enabling the plugin, the internal server errors happen. So I have now had to disable the plugin and I am considering using the other popular caching plugin because we have a popular site and customers are complaining too.
