invoice prefix year // header not correct

I have 2 issues:

#1 header not correct
right to the logo, it should show my company + address but:
– it’s not showing my company name (it’s filled in correctly)
– the column seems too small because it’s breaking my layout
– the country from client is not visible

#2 formatting invoice number
I would like to have an invoice format like WEBFACTUUR-2015-0000#
Where do I put the $year variable? When adding “Y” like the date format lower in config panel, it creates WEBFACTUUR-Y-0001 instead above example.
