Irregular Output

Some fields are displaying correctly, some are displaying incorrectly, some aren’t displaying and some that don’t exist are displaying!

Displaying incorrectly:
Street Address (displays “Street address (optional)” and “House number and Street Name”)
Street Address 1 (displays nothing and “Apartment, etc”)
Town / City (displays “Town / City (optional)”)
Postcode / Zip (displays “Postcode”)

Not displaying:
Routine medicine… (at the bottom of the form – unfortunately, I can’t upload images here!)

Doesn’t exist but is displaying:
County!!!

Whatever I do I can’t make this right. Can you help?
