Yes!

You can put something like this inside [fu-upload-form] shortcode:

[fu-upload-form]
[input type="text" name="country" description="country"]
[/fu-upload-form]

How can I change the order of the fields? For example I would like there to be other fields in-between the TITLE and DESCRIPTION fields? How can I do this or build a custom form?

The following works for my Custom Fields:

[fu-upload-form]
[input type="text" name="country" description="country"]
[/fu-upload-form]

But when I want users to upload an image to a Custom Field it doesn’t work:

[fu-upload-form]
[input type="file" name="country" description="country"]
[/fu-upload-form]

Is there a way to make this work?
