Is it possible to create a page from a form?

I’ve looked at several plugins and not found this so maybe it isn’t possible but I’d love to hear some feedback or maybe workarounds or maybe I’m not using the right terms.
What I’m trying to accomplish is to have a static page with a form that a logged in user could fill out and submit.
Then I want to use the fields from that form submission to create a new page (with a specific template I’ll have setup) and fill in some blanks with the info from the form fields.
