Is there a plugin I can use to create a product order form with check box?

    I just started creating a personal photography business website in WordPress using a Photocrati theme. I want to know if WordPress has an easy to use plugin that I can use to create a form which shows my different photography packages I offer A,B or C etc….and a check off box where clients can click on to choose which package they want to order. Also I want my form to able to show individual added services that the client can check off on the box and when completed, I get an email with the info they selected. Also an area where a client can fill in comments to add additional informtaion to the form. Adding a Paypal payment with the order would be a plus as well. I’m still searching for a reliable and easy to use plugin. Please comment with any information you can provide. Thank you.

    I’ll download and try it out. For the description of my services, I have it in bullet format listed in rows and was wondering if after typing it will show up as seen or clumped together as a paragraph?

    Questions????…..I just installed eshop plugin and it looks like something I will use. I’m a newbie and this is my 1st wordpress page and I dont screw things up. Question….I am using a Photocrati template which came with a shopping cart page upon install. After installing eshop I noticed a eshop shopping cart and also a shopping rates page was automatically created and added to my top header area. Can I remove the Photocrati shopping cart as well as the eshop shipping rate page since I will never be shipping any items out and only offering local services? Or is there a button to click on to remove those pages somewhere in the plugin settings area? Please reply before I delete the pages. eshop looks wonderful so far and as I use it, I know I’ll have more questions. Thank you.
