Is there a way to display a video included in the form?

    Have you tried the option which disables HTML filtering? There is an option on the plugin settings page which will keep the plugin from doing any HTML filtering.

    Dashboard > Settings > Google Forms

    Look for the checkbox which prevents HTML filtering. It might be on the Advanced tab.
