Is this possible to do with Forminator?

Hello,
My first question is if it is possible to put a Likert Scale.
My second question is the following:
My goal is to do a test with questions and answers and be able to insert it on my website. Also, I would like to connect it with Hubspot.
Each answer has a score,?for?example:
1. Do you feel affected by the time change over the last two weeks?
Answers:
Not at all: It is 0
Level: Is 1
Moderately: It is 2
Severely: It is 3
2. Have you felt affected by the change in weather over the last two weeks?
Answers:
Not at all: It is 0
Level: Is 1
Moderately: It is 2
Severely: It is 3
If your answer to the first question is Level (1) and your answer to the second question is Moderately (2) your total score would be 3.
Based on those scores you will get a result.
For example:
From 0-3: You are depressed
From 3-6: You have to exercise
….
I WANT TO KNOW IF THIS IS POSSIBLE TO DO WITH YOUR PLUGIN OR IF IT POSSIBLE IN SOMEWAY?
