Is User Registration Broken

For the past week or so I have had an error on the display within the dashboard:

We just checked the form you have sent and registered a user without any issue. Regarding the display issue, would you please try checking for any conflicts with other plugins you have in your site. We were unable to see such an issue over here. Please check the screenshot below for reference.
