Thank you for contacting WooCommerce support.

I understand you’re experiencing an issue where items occasionally appear duplicated on bills when customers use OpenPOS to check out. This happens inconsistently, with no clear pattern, and varies in frequency.

A similar issue was also reported some time ago but since this happens rarely we were not able to find the cause of the issue.

Could you share screenshots of the complete Order Notes for the affected transactions?
These might provide clues to help identify the root cause of the issue.
