Issue with permalink settings?

Have there been any reports of issues with this plug-in reseting permalink structure? We have this installed on a very large site with hundreds of pages and have started experiencing the permalink settings being automatically changed back to plain (we have them set to post name usually). It has happened twice now in the past week, seemingly randomly, when a page is saved. We’re just trying to rule out possibilities. Thank you for your help!
