Issue with uploading

Hello,
I am using this plugin from a long time but today sudden its not working properly. Its not uploading the file and If file upload there is no download link for it. Then I try to upload from ftp and add the file but its give me simple id with file name, not giving me shortcode as given before. After adding the id counter dont work.

Can you please help me out.
