Issues with a single form

Hi,

Contact Form Version 4.7.

I have an issue with a new form on a website. It’ only the case with that sigle new form, my old ones still work normally. The form submits all information via email as desired.

The issues are:
1.) The sidebar is not at the side, but at the bottom.
2.) There is no message after hitting send (neither if it sent succesfully or if there is an error.)

I have no idea what the problem is, the forms are identical aside from the contents.
