Issues with Displaying Correct Results

The quiz is a 150 question gifts test. So each question a user must select from Never (0 points), Rarely (1 point), Sometimes (2 points), Usually (3 points), Almost Always (4 points). It’s not a right or wrong but points are tallied based on 30 different categories. What should happend when the user hits submit is a list of how many points they scored in each category but I get this:
