iThemes Security Keeps Breaking htaccess

This IP address isn’t actually invalid. It is something called IPv4 mapped IPv6 addresses. https://en.wikipedia.org/wiki/IPv6#IPv4-mapped_IPv6_addresses

It sounds like while your server understands that syntax, but your version of Apache was compiled without support for it enabled: https://httpd.apache.org/docs/2.4/bind.html#ipv6

You’ll unfortunately need to contact your hosting company to resolve this issue or turn off “Write to Files” in Global Settings. In that case, iThemes Security won’t add bans to your .htaccess file, but they will be still enforced by iThemes Security when WordPress loads.
