I was always annoyed by this plugin, because the original never gave me any problems regenerating thumbnails so why would I need to “force” it to? Also the poor english in the description of “REALLY force the regenerate thumbnail” put me off.
But after dealing with 69,000 images I checked what happened. Regenerate thumbnails leaves the old, unused thumbnails!
I wish it had an option to replace sizes in post content though, as I fear this will leave some 404 images. This could be fixed by an htaccess rewrite in your upload directory, but that’s a bit hacky!

As you can see all of the unused sizes have been cleaned up. Of course that means there might be some broken links in the post content, so keep that in mind as a trade-off!
