    The form isn’t written in JavaScript, it’s dynamically generated through PHP into HTML and JavaScript.

    You can place it using the shortcode or the widget. If you really need to put it somewhere else I cannot support that but you can find a link to the basic instructions in the FAQ under the heeding “Can I put the form elsewhere? (header, footer, sidebar without the widget)”.
