    This is what I see when I try to view the forms that dont work. There are some that do and some that don’t.

    Any Ideas? I have already tried to turn off all plugins and default theme…

    The form that doesn’t work doesn’t look like it was added correctly. There are no references to the form’s HTML in the page like there is on the form which works. You can quickly search for things like “ss-q-title” if you view the page source to see what I am talking about.

    It is possible the permissions on the first form aren’t set correctly within Google – you need to make sure the form is visible to anyone with a link otherwise the plugin cannot retrieve the form from Google.

    You may also want to check the amount of inline CSS your pages have – it is significant.

    Permissions are all set properly and, other than the differences in the form url, everything on the 2 pages is the same.

    I do see that the HTML is not loading for the form that is not appearing.

    Any other thoughts?

    Looks like you changed something because the form is loading now.
