Jetpack Contact Form not working

The Jetpack 2.2 Contact Form is not working on 3.5.1 Twentytwelve site with a child theme: https://karencaseyfusedglass.com/contact/
The site has Akismet and Jetpack plugins only, and I have disabled several of the Jetpack options. Tests do not show up in Feedback.
Any help is appreciated.
