Jetpack will not add a form

The “Add this form to my post” button does absolutely nothing for me.

I keep it as simple as possible:

1) go to a new blank page with nothing edited but the title

2) hit the jetpack “Add a custom form” image/button

3) hit the “Add this form to my post” button

It does not hang or crash. It just does nothing.

I can add fields, move them, exit out of the “Add a custom form box”. But it refuses to actually save a jetpack form.

Any help would be appreciated.
