Join Email List Form in Sidebar?

I’m looking for a way to add a simple “Sign Up for the Email Newsletter” form to my sidebar, where visitors enter their email address, which is then sent to me. This site has an example of what I’m looking for over on the left-hand side:
(…although now that I actually enter an address in that form, I notice that the functionality is not quite what I’m after. I’d like the user to enter their email address, hit “Go”, or “Send”, and get a small graphic indication that their email has gone through.)
Anyway, I’ve spent several days looking for a solution to this and have struck out so far. Anyone have any ideas? thanks!
