Judging spreadsheet missing second and third judge values

    Based on your recommendation, I rolled back the EWZ-rating code back to 1.1.25 and the Rating Forms were visible again for the judges (3) and their judging. No issues were reported to me by any judges. After I have closed the Rating Form, I have downloaded the scoring spreadsheet (one row per item), but it contains just a single judge score and a single judge comment, not the three expected. There are two blank columns after Score and Comment. I have review my setup and it is the same as previous.

    I did notice that there was an additional column Judge ID that I had not seen previously in the spreadsheet.
