Kubrick Guestbook problem

Hi, I have installed the kubrick guest book, https://www.craigatkinson.info . It works fine but there is only one input box for comments, I’d like another with email address and one for web address and one for name…can anyone help please? I just used the code from that geeksmakemehot site
